Eligibility Determination Application

Single Point of Entry

As a designated Community Developmental Disability Organization (CDDO), Big Lakes is the single point of entry for services delivered to those who reside in Riley, Geary, Clay and Pottawatomie counties in Kansas. Our CDDO Quality Assurance/Eligibility Specialist can provide you with details on program qualifications as well as the types of services that are available in our area for individuals who are mentally retarded or otherwise developmentally disabled.

General Admission Criteria

This agency complies with Title XX of the Civil Rights Act of 1964. Big Lakes Developmental Center, Inc., as CDDO, uses the eligibility criteria as outlined in state policy, which apply to allAdmissions Meeting individuals seeking admission into any of the programs administered by the CDDO.

Each person over 18 years of age and not adjudged by the court to be incompetent shall be considered to possess legal capacity and shall be responsible for signing forms for him/herself whenever possible.

The CDDO Quality Assurance/Eligibility Specialist will serve as the initial point of contact for individuals seeking admission into one of the programs or services available in the CDDO service area.

PROCEDURES:

1. No individual will be excluded from services on the basis of sex, handicap, race, religion, color, age, ancestry, or national origin. However, the CDDO shall comply with a determination by the SRS Secretary that the applicant is inappropriate for community services because s/he presents a clear and present danger to him/herself or to the community.
2. Priority will be given to applicants who are from or who originate from the CDDO service area of Clay, Geary, Pottawatomie, and Riley counties of Kansas.
3. Applicants who meet the definitional criteria contained in K.S.A. 39-1803 and according to current state policies shall be considered eligible for admission into a program or service.
4. Applicants must have mental retardation or a developmental disability pursuant to the definitions contained in K.S.A. 39-1803 and according to current state policy which define:
"Mental retardation" means substantial limitations in present functioning that is manifested during the period from birth to age 18 years and is characterized by significantly sub-average intellectual functioning existing concurrently with deficits in adaptive behavior including related limitations in two or more of the following applicable adaptive skill areas:

 

1. Communication
2. Self-care
3. Home living
4. Social skills
5. Community use
6. Self-direction
7. Health and safety
8. Functional academics
9. Leisure
10. Work

A "developmental disability" is defined as a severe, chronic disability of a person that: a. is attributable to a mental or physical impairment or combination of mental and physical impairments;
b. is manifested before the person attains 22 years of age;
c. is likely to continue indefinitely; d. results in substantial functional limitations in three (3) or more of the following areas of major life activities:

 

i. self-care,
ii. receptive and/or expressive language,
iii. learning,
iv. mobility,
v. self-direction,
vi. capacity for independent living, and,
vii. economic self-sufficiency; and,

e. reflects the person's need for a combination and sequence of special, interdisciplinary, or generic care, treatment, or other services which are of lifelong or extended duration and are individually planned and coordinated.
5. Individuals referred from Unified School Districts (USD) for transition services must meet the eligibility requirements contained herein.
